I understand that martial arts training involves physical activity and carries inherent risks of injury. I voluntarily assume all risks associated with participation in Oliver Martial Arts Academy programs and activities.

In consideration of participation, I release Oliver Martial Arts Academy, its owners, instructors, and employees from any and all liability for injury, loss, or damage arising from participation in any program, class, or activity.

I agree to indemnify, defend, and hold harmless Oliver Martial Arts Academy and its staff from any claims, damages, losses, or expenses (including attorney's fees) arising out of or resulting from my child's participation in academy activities.

In the event of an emergency, I authorize Oliver Martial Arts Academy staff to seek medical treatment for my child if I cannot be reached. I acknowledge that I am responsible for all medical expenses incurred.

I grant Oliver Martial Arts Academy permission to photograph and/or video record my child for promotional purposes including social media, website, and printed materials.

I understand that if my child needs to take a break from classes, a written 30-day notice is required. If a break is not properly scheduled, I remain responsible for tuition. Breaks may be granted at 50% tuition at the discretion of management.
